Start with a moment you want to remember

An ADHD symptom tracker can be a place to keep notes about your day: when you struggled to start a task, what interrupted your focus or a question you want to ask at your next appointment. A short description can give you something specific to refer to later.

What to write down

Pick a moment that matters to you. Include enough detail that you will recognize it a week later:

  • When it happened. Add the date and an exact or approximate time.
  • What you were doing. “I reread the same email several times” is more specific than “bad focus.”
  • How it affected your day. Did you leave something unfinished, change your plans or need a break?
  • What you want to ask. Keep a question alongside the note so you do not have to think of it again at the appointment.

You do not need to fill every field or reconstruct days you missed. If you miss a day, you can start again with your next note. Leaving a day blank does not mean you were symptom-free.

Keep your notes together in EntreConsultas

Your health data stays on your device. No account is needed, and you decide what to export and share. You can add a medication manually if it is not listed. The built-in catalog currently covers Brazilian products.

Describe a change without deciding what caused it

“I felt restless during my afternoon meeting” records an experience. “My medication caused it” adds a conclusion that the note alone cannot establish. If you suspect a connection, keep it as a question for your clinician.

These are personal notes, not an ADHD assessment or a treatment score. EntreConsultas does not diagnose ADHD, decide whether a medicine is working or recommend dose changes.

Choose what you want to discuss first

Before your appointment, look through your notes and choose the concerns you most want to discuss. Keep the other entries available if you need details. The NIMH suggests preparing questions and a medication list before a mental health appointment, and being specific about your experiences.
